Virtual - Your Health Plan to Age in Place
Join the National Aging in Place Council - Chicagoland chapter to learn about building a health plan which will allow seniors to age in place. This presentation will provide an overview of the essential steps for in-home personal care and companion care services. It will also cover the importance of Patient Advocacy along with a Care Management Plan for successful aging in place for you and your family.
The National Aging in Place Council - Chicagoland chapter's mission is to educate and empower seniors and their families in the Chicagoland area to live safely, independently and comfortably in the home of their choice for as long as they choose.
This will be a virtual program conducted over Zoom. Registration is required and is limited to 100 participants. The program link will be emailed to registrants the day of the program.
